Brain scans reveal a possible cause of long COVID fatigue and brain fog

Millions of people worldwide continue to struggle with the lingering shadows of COVID-19, grappling with a crushing exhaustion and cognitive haze often described as brain fog. For years, these symptoms remained frustratingly elusive to medical science, leaving patients feeling invalidated and without clear treatment options. However, a groundbreaking study from the Centre for Addiction and Mental Health has provided some of the most compelling evidence yet that long COVID may actually be causing physical damage to dopamine-releasing neurons in the brain.

Using advanced PET imaging technology, researchers discovered significantly lower levels of markers associated with healthy dopamine neurons in patients suffering from long COVID. These deficits were concentrated in the striatum, a critical region of the brain responsible for regulating everything from basic movement to complex thinking and motivation. Interestingly, the location of the neuronal loss seemed to mirror specific patient struggles; those with losses in certain areas experienced more severe memory lapses, while others suffered from a profound lack of drive or slowed physical movements.

This discovery builds upon previous research which indicated that long COVID triggers unusual levels of inflammation in the brain. Scientists now believe that this persistent inflammation likely attacks and destroys dopamine neurons over time. According to lead researcher Dr. Jeffrey Meyer, this mechanism explains why many patients feel a sudden shift in their personality or capability, moving beyond simple tiredness into a systemic failure of the brain’s reward and motor systems.

For patients who have spent years searching for answers, these findings offer more than just scientific data; they provide essential validation that their invisible illness is rooted in biological reality. This breakthrough is already shifting the conversation regarding potential cures. Rather than focusing solely on general inflammation or immune responses, doctors are now looking toward medications that can boost dopamine function or prevent further metabolic breakdown in these vital neurons.

Looking ahead, the research team is preparing to launch a clinical trial in partnership with the University Health Network to test whether targeting dopamine activity can effectively reverse fatigue and restore cognitive clarity. While it may take time to refine these therapies, the transition from wondering why soon recovered patients remain sick to seeing the actual cellular damage marks a pivotal turning point in the fight against long COVID.
