IMS: Fezolinetant’s improvements in sleep quality, hot flashes as early as week 4

Women struggling with the disruptive effects of menopause may find relief much sooner than expected thanks to new real world data regarding the nonhormonal medication fezolinetant. Findings presented at the 20th World Congress on Menopause by the International Menopause Society reveal that patients experienced significant improvements in hot flashes and night sweats as early as four weeks into treatment. These results come from a preliminary analysis of the OPTION VMS study, which tracked women aged 40 to 75 receiving various nonhormonal therapies in routine clinical settings.

Beyond just reducing heat flashes, the drug showed a powerful impact on one of the most debilitating aspects of menopause: sleep deprivation. Researchers used a combination of patient reports and medical grade wearable devices to track rest patterns, finding that participants saw substantial gains in sleep efficiency and fewer nighttime awakenings starting at the fourth week. By the twelfth week of treatment, some users reported an average reduction of two point six nighttime awakenings compared to where they started. This is particularly significant given that up to sixty percent of menopausal women suffer from sleep disturbances, often waking frequently due to intense night sweats.

Dr. Marla Shapiro of the University of Toronto emphasized how reassuring it was to see objective data from wearables align perfectly with what patients were reporting subjectively. She noted that because hot flashes and insomnia have such a far reaching impact on a woman’s daily quality of life and general well being, having concordant evidence of improvement provides strong validation for the treatment’s efficacy in a real world environment outside of tightly controlled trials.

From a technical standpoint, fezolinetant works as a selective neurokinin 3 receptor antagonist, essentially blocking specific signals in the brain to help stabilize the body’s internal thermostat located in the hypothalamus. Throughout the observation period, these benefits remained steady through twelve weeks without any new safety concerns emerging during the preliminary analysis. While some mild liver enzyme elevations were noted in two rare cases, neither resulted in a patient needing to stop their medication, suggesting a favorable profile for those seeking alternatives to hormone replacement therapy.
