Why rheumatoid arthritis pain may persist even when inflammation is under control

For years, doctors treating rheumatoid arthritis have focused primarily on quenching the fire of inflammation to relieve patient suffering. However, a growing body of research from Semmelweis University suggests that for a significant minority of patients, the pain does not disappear even when medical tests show that inflammation is under control. This phenomenon leaves between six and twenty eight percent of patients in a frustrating category known as difficult to treat, where traditional medications fail to provide lasting remission despite clinical markers showing improvement.

The researchers found that persistent symptoms are often fueled by factors entirely separate from the autoimmune attack on the joints. Issues such as depression, obesity, smoking, and sleep disorders do not just coexist with rheumatoid arthritis; they actively maintain the experience of pain. This creates a punishing vicious cycle where chronic pain leads to poor sleep and lower mood, which in turn reduces physical activity and contributes to weight gain, further intensifying the sensation of pain and making daily life an uphill battle.

To combat this, the team has introduced a new diagnostic model that encourages physicians to look beyond standard inflammatory markers. Rather than automatically increasing medication doses when a patient continues to suffer—which can lead to unnecessary side effects—doctors are urged to investigate these secondary drivers. By recognizing when objective measures of disease activity have improved while subjective pain persists, clinicians can pivot their focus toward managing mental health or lifestyle factors that may be mimicking or amplifying the disease symptoms.

This holistic approach is gaining international traction and is now being applied to various other chronic conditions globally. Looking forward, the team at Semmelweis University hopes to integrate artificial intelligence into their methodology. By using AI based pattern recognition, they aim to identify specific subgroups of patients more accurately, allowing for truly personalized treatment strategies that address both the biological炎症 and the complex psychological and physiological hurdles that keep patients in pain.
